1. Scope
This Privacy Policy applies when you use the Unity Medical Transit website, request a quote or ride, communicate with us, receive transportation services, or use an authorized customer, facility, driver, dispatcher, or administrator portal (collectively, the “Services”).
2. Information we collect
Depending on how you use the Services, we may collect:
- Contact and account information: name, email address, telephone number, organization or facility, login and authentication information, and communication preferences.
- Transportation information: passenger initials, pickup and destination addresses, service date and times, mobility or assistance level, trip status, driver and vehicle assignments, and notes you choose to provide.
- Quotes, billing, and payment records: quoted and final fares, discounts, deposits, balances, invoices, payment status, refunds, and payer information. Payment-card details are entered into and processed by Intuit/QuickBooks or another payment processor; Unity’s operations application is not intended to store complete card numbers or security codes.
- Driver and workforce information: work schedules, time punches, inspection records, training acknowledgments, and on-duty device location used for dispatch, safety, route progress, and estimated arrival information.
- Communications and consent: messages, emails, call or text preferences, SMS consent records, confirmations, reschedule requests, and support communications.
- Technical information: device, browser, IP address, application activity, timestamps, audit records, security events, and similar diagnostic information.
Please do not submit diagnoses, Social Security numbers, full medical records, insurance-card images, or payment-card information unless Unity specifically requests the information through an approved secure process.
3. How we use information
- Provide quotes, confirm availability, book and coordinate transportation.
- Dispatch drivers and vehicles, provide routing and arrival information, and update trip status.
- Communicate about quotes, deposits, appointments, scheduling, driver arrival, changes, cancellations, and customer service.
- Create invoices, record payments and refunds, reconcile accounts, and maintain business records.
- Manage authorized accounts, workforce records, vehicle inspections, safety, training, compliance, and quality assurance.
- Protect the Services, investigate errors or misuse, maintain audit history, and meet legal obligations.
4. SMS and email communications
If you provide a mobile number and affirmatively consent to text messages, Unity may send transactional messages about your quote or transportation request, including confirmations, scheduling updates, payment links, reminders, driver-arrival notices, and customer service. Message frequency varies. Message and data rates may apply. Reply STOP to opt out or HELP for assistance. Consent to text messages is not a condition of purchase.
We do not share mobile information, SMS opt-in data, or consent with third parties or affiliates for marketing or promotional purposes. We may provide this information to messaging service providers only as necessary to deliver and support requested messages. We may also send service-related email to the address you provide. You may update your communication preference by contacting us.
5. When we disclose information
Unity does not sell or rent personal information. We may disclose information only as reasonably necessary:
- To authorized Unity personnel and contractors who need it to perform their duties.
- To a facility, payer, transportation partner, driver, or other party involved in arranging or providing the requested service, using the minimum information reasonably necessary.
- To service providers supporting authentication, hosting, communications, forms, maps and routing, GPS, document storage, payments, accounting, security, and support. These may include Google, Jotform, Supabase, Quo, Resend, Intuit/QuickBooks, and their replacements or subprocessors.
- To comply with law, a valid legal request, enforce our terms, protect rights or safety, investigate fraud, or complete a business transaction subject to appropriate safeguards.
6. Health and sensitive information
Some transportation information may be sensitive and, in certain circumstances, may be protected health information. When applicable, Unity uses administrative, technical, and physical safeguards and handles protected information in accordance with applicable law and contractual requirements. The Services are not a patient portal and should contain only information reasonably needed to arrange and provide transportation.
7. Location information
With device permission, the driver application may collect location while a driver is on duty or actively using location-enabled dispatch features. Unity uses it for live fleet visibility, pickup navigation, trip progress, safety, and estimated arrival information. Device and operating-system settings may limit location collection, but doing so can reduce these features. Facility and customer portals do not continuously track a customer’s location.
8. Data retention and security
We retain information for as long as reasonably necessary to provide the Services, maintain operational and financial records, resolve disputes, enforce agreements, and satisfy legal or contractual requirements. Retention periods vary by record type. We use reasonable administrative, technical, and physical safeguards designed to protect information, but no system is completely secure.
9. Your choices and rights
You may ask to review or correct contact information, change communication preferences, or request deletion of information that Unity is not required to retain. Depending on applicable law and your relationship with Unity, additional rights may apply. We may verify your identity before acting on a request, and some operational, safety, payroll, billing, audit, or legal records cannot be deleted immediately.
10. Children
Our online Services are not directed to children under 13. A parent, guardian, facility, or other authorized adult should arrange transportation for a minor.
11. Third-party services and links
Third-party services have their own privacy practices and terms. Unity is not responsible for an external service’s independent practices. Review the provider’s notices before submitting information or completing payment on its website or application.
12. Changes to this policy
We may update this policy as our Services or legal obligations change. We will post the revised policy with a new “Last updated” date and provide additional notice when required.
